REPORTING REQUIREMENTS

 




GRANT RENEWAL GUIDELINES

The Irene S. Scully Family Foundation will consider renewal proposals from organizations interested in additional funding. We strongly recommend that all organizations discuss the possibility of a renewal with one of the foundation's program officers prior to submitting a proposal.

 

Please include the following in your Grant Renewal Proposal:

 

  1. Newly completed versions of the following forms:
  2. Summary of your organization's previous funding relationship with the Irene S. Scully Family Foundation.
  3. Statement of Need - describe need for renewed funding.
  4. Expected outcomes - describe expected outcomes as a result of received funding.
  5. Evaluation and Assessment - describe how your organization will assess results and outcomes of the funded project/program.
  6. Annual operating budget.
  7. Project/program budget (if applicable).
  8. List and status of current revenue sources.
  9. Proof of 501(c)(3) status.
  10. Interim Report associated with previous Irene S. Scully Family Foundation grant.
  11. Final Report associated with previous Irene S. Scully Family Foundation grant.
