RENEWAL GUIDELINES
We ask that all grantees adhere to the reporting and renewal inquiry deadlines indicated in their award letters. Renewal inquiries will only be considered when funds from the previous year’s grant have been fully utilized and a final report has been submitted to the Foundation. Please note that grantees interested in a renewal must submit an LOI and Final Report forms to the Foundation by the LOI deadline for the cycle they wish to be considered in.
Please submit the following:
- Completed Applicant Information Form.
- Summary of organization’s history with the Foundation.
- Logic model. Multiple logic models may be appropriate for organizations applying
for funding for more than one program. Organizations may use the downloadable
Logic Model Template if needed.
- Demographic profile of community served by your program(s).
- Detailed description of program(s).
- Explanation of how your organization will assess and evaluate its work.
- Organization budget and program budget(s) (if applicable).
- List and status of current revenue sources.
- Proof of organization’s 501(c)(3) status.
- Copy of Interim Report and Final Report.
