PROPOSAL GUIDELINES

Please submit the following:

• Completed Applicant Information Form.
  
• Organization summary: describe the nature of your organization and its role in the community.
  
• Logic model. Multiple logic models may be appropriate for organizations applying for funding for more than one program. Organizations may use the downloadable Logic Model Template if needed.
  
• Demographic profile of community served by your program(s).
  
• Detailed description of program(s).
  
• Explanation of how your organization will assess and evaluate its work.
  
• Organization budget and program budget(s) (if applicable).
  
• List and status of current revenue sources.
  
• Proof of organization’s 501(c)(3) status.
